  1. How do I make an appointment?

You may make an appointment online at www.sb420doctor.com/appointment.htm or by phone at 818-894-4200.

  1. What do I need to bring on my appointment?

Patients should bring their California state issued ID, name and contact information for your primary physician, list of current medications and your medical records identifying the condition for which you are seeking recommendation.

  1. What happens when I visit our clinic?

You will be asked to fill out an intake sheet including prior medical history, family history and social history.  An interview and physical examination will be done to determine if medical marijuana is an appropriate treatment for your symptoms.  If medical marijuana is felt to be appropriate, a treatment plan including periodic follow up evaluations will be given as well as a provision of specific forms for dispensary use.

  1. How do I obtain my medical records?

Medical records can be requested from your primary doctor.  The doctor’s office will ask you to sign a release of medical records form to obtain a copy for your evaluation.

  1. Do I need to be referred by another physician?

No, but you must have a qualified medical condition and medical records noting your symptoms.

  1. What do you receive if a recommendation is given?

You will receive a physician statement as well as a patient identification number used for Phone and Internet verification.

  1. How long will the recommendation be good for?

Medical Marijuana recommendations will be good for no more than one year and may be less than one year as well.  All follow-up visits within the first year are free.

  1. If I receive a recommendation, will I have to return to the clinic?

Yes, the doctor is required to have a treatment plan which will vary depending on the nature of the serious medical condition.

  1. Does the clinic provide Disabled Access?

Yes.

  1. What forms of payment do you accept?

At this time, we accept cash as the only form of payment.

  1. What is the state ID program?

  1. If I receive a recommendation, where and how will I get my medicine?

Unfortunately, we are not able to discuss how and where to find your medical marijuana.  Please do not ask staff about dispensary locations.
